The oral examination represents the culmination of the initial certification journey, following completion of a neurosurgical residency in an ACGME accredited program and passing the written examination. While the written examination assesses knowledge across various disciplines, the oral exam evaluates clinical expertise and decision-making in neurosurgical practice after an applicant has been an independent practitioner. Conducted through face-to-face sessions with current and former board directors and guest examiners, this assessment covers preoperative evaluation, intraoperative decision-making, and post-operative management, including handling of complications. The American Board of Neurological Surgery oral examination undergoes regular review and enhancement to ensure relevance and rigor.
